  • A 18-year-old female c/o: Fever, loss of appetite and loss of weight since 1 month.
  • CXR: Mediastinal widening.
  • HRCT thorax showed Conglomerated mediastinal & right hilar lymphadenopathy.
  • Sputum AFB was negative and sputum Genexpert MTB not detected.

 

  • FOB-TBNA( subcarinal LN) 
  • CB-NAAT – MTB detected, Rifampacin sensitive.
  • Patient  took drug sensitive anti tubercular treatment for 9 months

 

  • After that, patient c/o Diminution of vision in left eye.
  • Slit Lamp examination-anterior uveitis with choroidal granuloma.
  • Started on steroids – topical & oral 

 

  • After 1 month, repeat HRCT thorax revealed right hilar lymphadenopathy. 
  • Repeat FOB­-TBNA done.
  • CB-NAAT revealed M.TB with Indeterminate Rifampicin sensitivity. 
  • Started on Shorter oral Bedaquiline regimen. 

 

  • Patient completed a course of MDR regimen for 9 months.
  • Hrct repeated was s/o resolution mediastinal lymph nodes.
  • Ocular examination was s/o healed choroidal granuloma with no active lesions.
