PULMONOLOGY

	<li>A 18-year-old female c/o: Fever, loss of appetite and loss of weight since 1 month.</li>
	<li>CXR: Mediastinal widening.</li>
	<li>HRCT thorax showed Conglomerated mediastinal &amp; right hilar lymphadenopathy.</li>
	<li>Sputum AFB was negative and sputum Genexpert MTB not detected.</li>

	<li>FOB-TBNA( subcarinal LN)&nbsp;</li>
	<li>CB-NAAT &ndash; MTB detected, Rifampacin sensitive.</li>
	<li>Patient &nbsp;took drug sensitive anti tubercular treatment for 9 months</li>
</ul>

&nbsp;

<ul>
	<li>After that, patient c/o Diminution of vision in left eye.</li>
	<li>Slit Lamp examination-anterior uveitis with choroidal granuloma.</li>
	<li>Started on steroids &ndash; topical &amp; oral&nbsp;</li>
</ul>

&nbsp;

<ul>
	<li>After 1 month, repeat HRCT thorax revealed right hilar lymphadenopathy.&nbsp;</li>
	<li>Repeat FOB&shy;-TBNA done.</li>
	<li>CB-NAAT revealed M.TB with Indeterminate Rifampicin sensitivity.&nbsp;</li>
	<li>Started on Shorter oral Bedaquiline regimen.&nbsp;</li>
</ul>

&nbsp;

	<li>Patient completed a course of MDR regimen for 9 months.</li>
	<li>Hrct repeated was s/o resolution mediastinal lymph nodes.</li>
	<li>Ocular examination was s/o healed choroidal granuloma with no active lesions.
