Congenital Heart Disese

A 28-year-old woman presented with complaints of mild to moderate shortness of breath. The patient also had additional complaints of blue lips and hands and feet, abdominal pain. 
Past medical history: The patient came with complaints of moderate intensity shortness of breath since 1 week ago. Complaints of shortness of breath persist and are accompanied by body weakness. The face and hands and feet have been blue since 5 days ago. there are complaints of moderate intensity abdominal pain since 3 days ago. currently there is no chest pain, no cough and no fever RPD: History of shortness of breath since after delivery 1 week ago. 

Full consciousness, adequate contact, BP: 190/150 mmHg, HR: 92, RR: 25, SpO2: 77% room air, T: 36,5
Head: normocephal (+) circumferential cyanosis
Eyes: CA(-/-). SI(-/-). Light reflex (+/+)
Neck: normal (+)
Thorax: bilateral vesicular, sonorous +/+, bilateral symmetrical, regular BJ 12, basal pulmonary fine rales murmur( -), gallop( -)
Abd: BU +, soft, Epigastric tenderness +, tympanic +.
Acral: Warm, CRT < 2 seconds, acrocyanosis

Laboratory: Hb: 20,5, Leukcocyte: 9.000, Thrombocyte: 335.000

Emergency Hypertension 
Susp. CHD Cyanotic dd CAD 
Reactive polycythemia (?)

P/
NRM 02 15 lpm --> spo2: 88% (patient does not appear congested) 
IVFD RL 500 ml/24 hours 
Nicardipine drip 5 mg/hour 
Inj. Omeprazole 40 mg bolus IV
