A 27-year-old woman presented with symptoms of vaginal bleeding, urinary retention, abdominal pain, and a necrotic vaginal mass. Initial biopsies showed inflammatory exudates with necrotic tissue, but her condition worsened, so further examination and biopsies were done. Diagnostic laparoscopy revealed an inverted uterus with healthy ovaries and fallopian tubes. A laparotomy and vertical hysterotomy were needed to revert the uterus to its normal position. Further biopsies showed embryonal rhabdomyosarcoma (ERMS). She underwent a total hysterectomy with bilateral salpingo-oophorectomy and started VAC chemotherapy one month after surgery. Final pathology showed no residual disease, and the patient is currently without evidence of recurrence.
