A 31-year-old female presents with a two-week history of fever accompanied by productive cough and exertional dyspnea. On examination, her vital signs are stable, and her oxygen saturation is 98% on room air. Bilateral crepitations are noted on chest auscultation. Additionally, bilateral lower limb pitting edema is observed, along with rashes. Troisier's sign is positive. Blood investigations reveal potassium levels of 2.7, calcium levels of 2.4, and creatinine levels of 1.59 and Arterial blood gas analysis shows metabolic acidosis. Chest X-ray reveals bilateral consolidation. Widal, scrub, and sputum acid-fast bacilli tests are negative. Kindly suggest further management and diagnosis.
