A 35-year-old male presented with fever and tiredness for the past 5 days and a cough for 1 day. On detailed history, fever is more towards night, and it gets relieved by taking antipyretics.No history of drenching night sweats. It was associated with tiredness. Further, he noticed his shirts become loose, and he has lost around 8kg during the past 2 months No history of fever in his households. Since today he is also experiencing occasional cough, which is predominantly dry. No history of post-tussive vomiting, breathlessness, noisy breathing, shortness of breath while lying down, weakness of body parts, abdominal pain, distension, and vomiting. Past history of varicosities of the left leg since 6 years old. Occasional alcoholic 10 units per month, Non-smoker, no promiscuous sexual activity, no substance abuse. Works as an ornamental maker in Kakkacheri. Migrated from his native place agra to here years back in search of a job. O/e Pallor present, no clubbing cyanosis and edema. Varicosities noted in left lower limb extending down the popliteal fossa along lateral aspect S/e P/A soft NT no organomegaly CVS: S1S2 heard. RS: AEBE, no murmur, No FND.
