A 35 year old male with h/o Type 1 DM and is treated with an intensive insulin regimen. He uses a rapid-acting insulin injection before meals three times a day and one injection of isophane insulin at bedtime every night. His blood sugar in the morning is usually high. He complaints that he wakes up ‘tired’ and during the night he has nightmares. Can this be due to hypoglycemia early morning ? Suggest alternative to isophane insulin that has shown better results in such cases.
