A 45-year-old male has reported a two-day history of fever, with a sudden onset of shortness of breath since last night. He underwent hemi-arthroplasty 20 days ago. Current laboratory findings include a platelet count of 13,000, D-dimer level of 2.5, hemoglobin at 6, white blood cell count at 7,500, serum creatinine at 3.8, and blood urea at 146. An echocardiogram shows normal results, with a respiratory rate of 45 and oxygen saturation at 90% on room air. There is bilateral pedal edema without hypotension, and D-dimer elevation is not significant. Kindly suggest an appropriate diagnosis and suitable treatment options.
