A 48-year-old female presented with a complaint of pain in the left hypochondriac region. The patient has been experiencing a low-grade fever for the past 10 days, and the pain is exacerbated in a supine posture. There is no relation to respiration was noted but the patient noticed a loss of weight and appetite. There is no previous history of tuberculosis or diabetes. The patient has hypertension. Her vital signs are stable, and there is tenderness on deep palpation in the epigastric region, while the rest of the examination is normal. Her random blood sugar (RBS) level is 471 mg/dL, hemoglobin (Hb) level is 10 gm/dL, total leukocyte count (TLC) is 15,000/mm³, and C-reactive protein (CRP) level is 247 mg/dL. A high-resolution computed tomography of the chest has been performed, and the images are given below. kindly suggest an appropriate diagnosis and management.
