A 55-year-old male presented with skin bulging, redness, and crepitus on the right side of his chest, persisting for the past two months, as shown in the image. He complained of pain associated with the bulging skin. Over the same period, he reported experiencing fever and a non-productive cough. The patient also noted a weight loss exceeding 12 kilograms in the last two months and a decreased appetite. Vital signs indicated a blood pressure of 100/70 mmHg, heart rate of 113 beats per minute, respiratory rate of 25 bpm, and temperatures of 37.8 and 37.1 degrees Celsius, with an oxygen saturation of 91% on room air. The patient was suspected of having lung tuberculosis with subcutaneous emphysema, and cellulitis was considered as a possible alternative diagnosis. Treatment included oxygen therapy, antibiotics, and analgesics, while further diagnostic tests, including a gene-Xpert test and Chest CT-Scan, were scheduled to confirm the diagnosis.
