A 57-year-old male, intermittently using mirtazapine for generalized anxiety disorder for the past 20 years, recently ceased chlordiazepoxide and amitriptyline (taken for two months). In the last 15 days, he has experienced weakness and insomnia. The patient, hypertensive and on telmisartan 40 mg OD for a month, has a history of a road traffic accident five years ago. An MRI conducted a year later revealed lacunar infarcts in the left putamen. Additionally, the patient recalls childhood blackout episodes and is now asymptomatic. Family history includes sudden loss of consciousness, seizures, and death in the paternal uncle at 40-50 years. Kindly suggest possible diagnoses and differentials, along with the recommended management.
