A 60-year-old male presents with multiple tophi in the leg that began three months ago. The patient also reports pain around the thumb and ankle, which is localized and appears reddish. Despite initiating pain medication, there has been no relief. The current examination reveals elevated blood pressure at 140/80 mmHg, with normal heart and respiratory rates. Multiple tophi are observed around the leg, accompanied by swelling and inflammation. Laboratory results show a uric acid level of 11 and a creatinine level of 1.3. The patient has been admitted to the hospital and commenced on a colchicine loading dose. After pain resolution, allopurinol 300 mg has been initiated.
