A 68-year-old female presented with central chest pain, intermittent since this morning. Past medical history includes hypertension managed with Telmisartan 40 mg OD. Family history is significant for a mother who died of stroke at age 80, and the patient recently lost her husband to an acute myocardial infarction (MI) last week. Vitals: HR 105 bpm, BP 140/90, SpO2 99% on room air, GRBS 95. ECG shows sinus tachycardia, normal axis, and symmetric T inversions in V2-V4. TnI levels are elevated at 0.36 (Normal <0.04). A bedside 2D echo (apical 4-chamber view) is attached. Kindly suggest diagnosis and treatment.
