A 90-year-old patient presented with symptoms of breathlessness, fatigue, weakness, and difficulty sleeping, persisting for 15 days. On examination, dull chest percussion and creaking sounds were detected. Urticarial patches were observed on the patient's hands. Vital signs revealed a blood pressure of 90/60 mmHg, temperature of 98.8°F, SPO2 of 60%, and a pulse rate of 112 bpm. Blood investigations indicated leucocytosis (15,000), urea levels of 52, and haemoglobin levels of 10.8 gm. Liver function tests were normal, but there was neutrophilia. An X-ray revealed a bilateral pleural effusion involving two lobes on the right side. An ultrasound was conducted, which suggested hepatomegaly and fatty liver.
