A 19 year old male patient presented to Government General Hospital, Kurnool with complaints of sudden onset breathlessness and pain in the loin. On examination, his O2 sats were 88% with room air, Pulse rate: 124 bpm, ECG: Sinus tachycardia with S1Q3T3. USG abdomen has shown evidence of hyperechoic lesion of 1.2 x 0.9 cm inside the IVC partially occluding the lumen of IVC s/o thrombus, Wells score: 4.5; CTPA: Non enhancing filling defect in Left pulmonary artery. CECT abdomen: Non enhancing filling defect in IVC at insertion of left renal vein
