A 20-year-old female with a history of previous POTT spine surgery in 2021 presents with stiffness and pain in both lower limbs, immobility, and severe spasms affecting personal care. She underwent D10-L2 Fixation with spasticity of both lower limbs, bilateral lower limb contractures at Hip flexors & adductors and knee flexors with severe painful flexor spasms. No history of DM, HTN, or IHD. Preoperative laboratory findings showed Hb at 11.5 g/dl and a total count of 11250 cells/mm³. Biochemistry results indicated urea at 18.00 mg/dl, creatinine at 0.50 mg/dl, and sodium at 135 mmol/L. Microbiology findings indicated non-reactive results for HIV 1 & 2, HCV, and HBsAg. After informed consent, she underwent an intrathecal baclofen trial (50μg) pump installation at 75 μg per day on 06/03/2023, along with bilateral hip flexor, adductor, and medial hamstring release. Her postoperative course was uneventful. Postoperative laboratory findings demonstrated a haemoglobin level of 10.7 g/dl, total cell count of 11630 Cells/mm3, neutrophils at 80.7%, lymphocytes at 13.6%, and PCV at 32.8%. Biochemistry results indicated urea at 8.00 mg/dl, Creatinine at 0.60 mg/dl, and Sodium at 135 mmol/L. Instructions included wound care, the use of an air mattress, and leg traction were provided. Her vital signs were stable at discharge, and she was advised to follow up in the OPD on March 16th, 2023.
