In a 24-year-old primipara with hitherto normal dichorial diamnotic twin pregnancy premature labor occurred. After massive maternal pulmonary edema associated with tocolytic therapy two female infants were born by cesarean section at 28 weeks of gestation. Both neonates showed fast recovery after respiratory support. The rather big and asymmetric mouth of the first-born child was conspicuous immediate after birth.
