A year old Playboy turned

A 31 year old " Playboy " turned adult ( that's my way of saying he has history of multiple sexual partners in past) presents with inability to ejaculate and fever 4 days back and polyuria. He did an USG which perplexed me than doing anything useful to be honest. Only positive history findings were constipation, dyslipidemia , irritability ( me more than him tbh) and inability to ejaculate . Any urologists or good samaritan surgeon pls help me help him. PS : I hope he never sees this otherwise I'm dead meat Ps : I hope he never sees this
