A 35-year-old black female with type 1 oculocutaneous albinism and multiple basal cell carcinomas of the face visited the dermatology clinic to diagnose and treat painful lesions, multiple irregulars, rough and scaly macules on both vermilion borders of upper and lower lips, and margins as shown in the figure.
Microscopic examination showed epithelial changes of hyperkeratosis, acanthosis, dyskeratosis and moderate dysplasia, with a thick chronic inflammatory infiltrate and lamina propria basophilic degeneration. The diagnosis was actinic cheilitis. Given the extent of the lip lesions, surgery was decided. Microscopic analysis of the tissue withdrawn by vermilionectomy showed areas of severe dysplasia and basophilic degeneration. There was no proof of in-situ or invasive squamous cell carcinoma. The patient was advised about sunlight protection using adequate sunscreen preparation with high SPF. The healing was uneventful.
