Migraine and stroke exhibit a complex association, with both conditions having significant societal impacts. Stroke ranks as the second leading cause of death and the third most common cause of disability globally. Meanwhile, migraine affects approximately 13% of the population, with a higher prevalence in women of reproductive age. Emerging evidence links migraine to white matter lesions, cardiac events, and vascular diseases. Several cardiovascular comorbidities, such as hypertension and ischemic heart disease, are more prevalent in migraineurs, increasing their risk of stroke. Notably, migraine with aura carries a twofold risk of stroke, while the frequency of migraine attacks further influences stroke risk. Understanding the underlying mechanisms, including cortical spreading depression, vascular dysfunction, genetic factors, and cardiac abnormalities, is crucial to managing these conditions effectively.
