We report a case of 64 years old female who presented in emergency room with a history of on and off fever, uneasiness, chest heaviness from last five months and   altered mentation from last seven days. The patient had a history of Anti tubercular treatment  [ TB PLATINUM, INTERFERON GAMMA RELEASE ASSAY (IGRA) POSITIVE] but Anti tubercular treatment was with held due to ATT induced gastritis.
