A 51 year male hypothyroid , tobacco chewer hailing from hyderabad came with dyspnea class4 NYHA, orthopnea , PND, bipedal edema AND facial puffiness With low volume pulse , raised jvp in general examination .Apical impulse in 5th ICS 1.5cm lateral to MCL , with grade 2 parasternal heave and prominent epigatric pulsations with pansystolic murmurs in tricuspid and mitral areas with MDM murmur in tricuspid area and with wide fixed splitting of second heart sound. What will be the probable diagnosis and management ?
