A 26- years old female presented in emergency with severe abdominal pain, pallor and 130 bpm pulse rate with nonrecordable BP, tender abdomen and cervical motion tenderness.Patient came with USG of same day showing gestational age of 12wks at periphery and no other finding mentioned in USG .After examining the patient, planned for immediate laparotomy under general anesthesia with high risk consent. On opening abdomen 1.5 litres of blood was found in the peritoneal cavity &.foetus along with placenta was found in cavity. The per-operative findings revealed a ruptured communicating horn. Resection of the horn was done. The patient received 2 units of blood transfusion and postoperatively, remained stable. Notably, the patient had a previous normal delivery and there was no mention of any such issue in any previous reports
