<p>A 40-year-old housewife presented with chest discomfort with no prior history of diabetes or hypertension but had premature menopause at 37. She is obese, a former smoker, and lives a sedentary lifestyle. Her blood pressure is high (160/106 mmHg), and she has acanthosis nigricans on her neck. Further tests revealed high blood sugar, triglycerides (43.2 mg/dl), total cholesterol 149.4 mg/dl (125 to 200mg/dL), HDL 17.64 mg/dl (50mg/dL or higher), and LDL cholesterol 109.26 mg/dl (Less than 100mg/dL).In addition, she has macroalbuminuria, and her blood pressure remains persistently high despite treatment with non-steroidal anti-inflammatory drugs for costochondritis.</p>
