A 38-year-old man with a BMI of 33.2 kg/m2 and a waist circumference of 40 in. He presented to his primary care physician for a refill of allergy medication and mentioned that he was told he had high triglycerides (TGs) at a recent health fair. His father died of cardiac failure at age 50, and his mother has type 2 diabetes. His Triglycerides (220 mg/dL), High-density lipoprotein cholesterol (38 mg/dL), Blood pressure (140/90 mm Hg), Fasting plasma glucose (110 mg/dL) and his oral glucose tolerance test (OGTT) showed a 2-hour glucose level of 204 mg/dL, which was repeated two weeks later with the same result. He meets the American Diabetes Association criteria for diabetes based on the OGTT result, although his glycated haemoglobin is <6%. In addition to type 2 diabetes, the patient meets 4 of the five criteria for metabolic syndrome.
