A 49-year-old lady was admitted to the hospital with H/o hypertension controlled by oral medications. She complained of chronic neck pain, headache, and episodic vertigo and tinnitus for two years. The episode occurs for 1-2hrs twice daily.

Physical examination showed limited neck motion, slightly diminished sensation in both arms, and brisk deep tendon reflexes. In addition, she felt numbness of both upper extremities and weakness of all four extremities for one year. However, no specific trauma could be recalled.&nbsp;

Radiographic examination revealed mild diminution in the height of the C5/6-disc space (Left figure). In addition, an MRI scan showed cord compression by a large central herniation of the C5/6 disc with signs of myelomalacia. (Right figure)
