A 45-year-old patient taking insulin for two years was diagnosed with type 2 diabetes six years back. His body weight was 50 kg with a BMI of 24 kg/m2. 

He was presented with complaints of uncontrolled and increased blood glucose levels and a dramatic increase in insulin dose during the past five months without any apparent cause. He had no history of fever, infections, or steroid use. He had multiple hospital visits for the past few months with these complaints. Upon examination, lipo-hypertrophy along with swelling in the thigh region was established. After a thorough investigation of the patient, the physician found that he was injecting insulin on the medial aspect of his thighs.
