A 60-year-old man with adenocarcinoma of the colon underwent left hemicolectomy with primary anastomosis. Thirty hours after surgery, he was found to have a fever of 102°F (38.8°C), blood pressure of 90/60 mm Hg, heart rate of 140 beats/ min, respirations of 24 breaths/ min, and low urine output. Physical examination showed diffuse abdominal tenderness. The surgical site is clean and Gram stain did not show any organism. Urinalysis was negative and the complete blood count (CBC) showed leucocytosis.
