Fever with rash

An elderly lady presented to us with a complaint of new onset fever since last 5 to 7 days associated with new onset erythematous Maculopapular rashes since 3 to 5 days, non blanchable in nature and occasionally associated with itching. Upon routine evaluation his blood picture revealed bicytopenia in form of thrombocytopenia and leukopenia with neutropenia. He had a history of Rheumatoid arthritis and was on maintenance DMARDs... Methotrexate once a week and leflunomide once daily. Differentials please ?
