38 years old male, presenting with severe headache and continuous vomit for these 2 days. He felt excruciating pain with a pain score of 9/10. On physical examination, he has impaired motor function in both legs. Motor function test 3/3 for both legs, parese cranial nerve, and toungue deviation.
