A girl had a history of febrile illness as an infant, but the cause of the illness was unknown. The heart rate was 90 beats per minute. With a positive blood ASO titer, a 3-year-old female infant with a grade 2/6 systolic murmur over the precordium was referred for screening echocardiography. Figure 1 shows brilliant echoes on the tip of the pulmonary valve with mild thickening, indicating rheumatic inflammation, and Figure 2 shows mild pulmonary valve stenosis.
