A 65 year old gentleman presented with a right sided reducible inguinoscrotal swelling having an expansile cough impulse. A provisional diagnosis of inguinal hernia was made. After discussing various options with the patient surgery was planned. During surgery, the hernia sac was present antero medial to the cord structures and inferior epigastric artery was present medial to the sac.
