<p>A 65-year-old man has been experiencing right knee pain and fullness behind the knee for the past three months. He has received at least a couple of intra-articular steroid injections and has a known history of osteoarthritis in the knee. A big popliteal cyst can be seen behind the knee on sagittal and axial magnetic resonance imaging (MRI) (Figures 1 and 2).</p>
