An 86 years old male patient who has no known comorbidites and no addictions, presented with low back pain, abdominal discomfort and bilateral lower limb weakness of 4 months duration. He has past history of urinary hesitancy,  now well controlled with Silodosin. 
Usg abdomen suggested prostatomegaly, and Xray Lumbosacral spine was normal.
CBC showed thrombocytosis. So,  PET CT scan was done, which revealed metastatic prostatic carcinoma, later confirmed with Trephine biopsy.
The patient was referred to a Cancer hospital for further management.
