Spontaneous intracranial hypotension (SIH) is a debilitating syndrome resulting from spinal cerebrospinal fluid (CSF) leak attributed to factors like dural tears, leaking meningeal diverticula, or CSF-venous fistulas. With an estimated annual incidence of 3.7 per 100,000, SIH manifests as symptoms similar to intracranial hypotension from other causes, yet uniquely, the leak occurs spontaneously in the spine, with the precise site unknown at presentation. Characterized by orthostatic headaches and various neurological symptoms, SIH presents with MRI features of intracranial hypotension in approximately 80% of cases. Given its diverse presentations, SIH necessitates coordinated care across multiple medical specialties.
