A 25-year-old South Asian male presented with right upper quadrant pain associated with fatigue and headaches for six months. His physical examination revealed hepatomegaly and laboratory investigations showed elevated levels of serum alanine aminotransferase (ALT) 155 U/L and aspartate aminotransferase (AST) 59 U/L. Ultrasound examination of his liver revealed a hyperechoic texture, consistent with diffuse fatty infiltration. A liver biopsy revealed steatosis with lobular inflammation and hepatocyte ballooning without fibrosis, consistent with non-alcoholic steatohepatitis. Despite a calorie-restriction diet and exercise regimen resulting in weight loss and a reduction in BMI, the patient's symptoms persisted, and liver enzymes remained elevated
