A 34-year-old male patient with no abnormal physical examination and no history of alcohol consumption, diabetes mellitus, drug therapy or viral hepatitis C had a chest CT which revealed no pathological changes in the lung parenchyma but showed numerous variable-sized hypodense nodular foci in an unequal distribution in the liver parenchyma. Laboratory analysis data was not abnormal, and the APRI was 0.35 (negative predictive value for cirrhosis). Real-time shear wave elastography (SWE) data was obtained, and the stage of liver fibrosis was assessed using the METAVIR F0-F4 scale in accordance with the recommendations of the European Federation for Ultrasound in Medicine and Biology. Contrast-enhanced abdominal MRI was performed, which showed similar findings to the CT, and a quantitative assessment of the fat in the liver was carried out, which showed a reading of 34% corresponding to grade 2 (moderate fatty hepatosis). The lesions demonstrated no contrast enhancement, diffusion restriction, or mass effect from the foci. Further evaluation of the nodules and laboratory profile was planned.
