parkinsons disease

A 74 year old male weighing 65 kg was presented with abnormal rhythm movement of upper and lower limbs since 1 year.It started ok the distal part of both upper limbs simultaneously. It was noticed more during rest that not aggravated during movements. Recently the lower limbs were also affected was was noted more on both the great toes. Patient also gives history of difficulty in walking and stoops forward with small and fast steps.There is also decreased swinging of hands. No history of fall injury, memory impairment , altered sensorium, behavioural change, visual complaints, swallowing difficulty,weakness of one half of the body. Alcohol: occasional PR: 78bpm Gait: shuffling gait with tendency to fall forward and backward Body build: thin Consciousness: conscious GCS-15/15 Abnormal facial expression and secretion if salaiva Mask like face present , cogweel rigidity and resting tremor RBS : 138.1mg/dl Diagnosis: Parkinsons disease Suggest a suitable treatment plan
