A 62-year-old man had complained of painless hypertrophy of the nasal tip with irregular contours of the skin that had developed over 15 years. An ear, nose, and throat examination revealed that the enlargement and hypervascularity had formed on the external surface of the nose. The external nasal bulk with irregular tissue caused severe disfigurement and difficulty in nasal breathing. There were also multiple superficial pustular and inflammatory lesions of the rosacea on the nose. Due to the extent of the disease, the rhinophyma distorted the cosmetic appearance, as shown in the figure.

The excision of rhinophyma was performed under general anaesthesia. A radiofrequency electrosurgical unit was used in the cut/coagulate mode at 40 W. Thin layers of tissue were removed until the natural nose shape was re-created. Bleeding spots were carefully electro-coagulated with a ball electrode, spaced at 2 mm per 6 inches. A thick basis with 2% fusidic acid ointment and a non-adherent bandage was applied to the treated surface. A broad-spectrum sunscreen was applied.
