62/M presented with complaints of boggy mass, associated with purulent discharge from both eye since 2 mths. Patient also had complaints of reduced movement of left eye and reduced vision of left & right eye. Pt. however had no other asso. comorbidities. Biopsy from left eye was done showed no atypical cells and showed diffuse plasmalymphocytic cells, with diffuse fibrosis - was sugg. of IgG4 related disease. S. IgG4 done we're highly elevated, S. IgG done also were elevated. Pt was started on steroids, a had significant improved vision.
