73 yr old otherwise healthy male presented to opd with concerns about uncontrolled blood sugar levels for past 6 months for which he was on 3 OHAs along with insulin basal. He brought along almost all routine investigations which were WNL except Hba1c 13. After enquiries about compliance and insulin storage / adm I planned a USG w/abd and amylase/lipase(wnl) which showed mass pancreatic tail. Ca markers were raised, CT confirmed the diagnosis. Usg FNBx revealed Adenocarcinoma. Referd to Onc after PET wid no sec planned for CT+_RT.
