A 42-year-old male patient who had been diabetic and hypertensive for 6 years had contracted COVID-19 infection 2 months before and had been hospitalized for ARDS and kept on ventilatory support for 3 weeks. Tocilizumab 162 mg subcutaneous single dose and intravenous Dexamethasone 4 mg twice daily for 10 days, followed by oral dexamethasone tapering over a month.&nbsp;

He had developed acute kidney injury and urosepsis after recovering from pneumonia and being discharged from the hospital, necessitating readmission. During the 2-week hospital stay, tests revealed pancytopenia, increased serum creatinine, a Candida Albicans positive blood culture, and a Klebsiella Pneumoniae positive urine culture.

He was given IV fluids, antibiotics, antiemetics, antifungals, and supportive medication. He was discharged when he was systemically stable. During his two weeks in the hospital, he had noticed a gradual, painless deterioration of vision in both eyes.

Presenting Symptoms

He presented with blurred vision in both eyes. His BCVA was counting fingers close to his face in both eyes. The evaluation of the anterior segment was unremarkable. A fundus examination of both eyes revealed clear vitreous with multiple yellowish-white, fluffy lesions ranging in size from 1/2 to 1 disc diameter scattered primarily across the posterior pole.

The foveal centre was affected by the retinitis lesion in the left eye but was only marginally spared in the right. There were no retinal haemorrhages or pigmentary changes observed. In both eyes, the inferotemporal periphery from 3 to 6 o&rsquo;clock had evidence of moderate choroidal detachment. In both eyes, the retina was attached.
