A 45-year-old perimenopausal woman with infrequent menstruation for three years and a low anti‑Mullerian hormone (AMH) level of 0.4 ng/mL came to a fertility clinic. She opted for autologous bone marrow-derived stem cell therapy (ABMDSC) to rejuvenate her ovaries and increase her chances of pregnancy without donor oocytes. She underwent a bone marrow aspiration to collect stem cells, which were then laparoscopically instilled in her ovaries due to the small size and inaccessibility of the ovaries. After eight weeks of ABMDSC therapy, the patient's ultrasound revealed improvement with two follicles in each ovary and an improved AMH level of 0.9 ng/mL. Instead of opting for a second ABMDSC therapy session, she chose to go for a frozen embryo transfer, which resulted in a successful pregnancy. She delivered a healthy baby girl via cesarean section at 38 weeks.
