A 30-year-old woman presented with complaints of pelvic pain and irregular vaginal bleeding for the past three months. She also revealed a history of a miscarriage at nine weeks about eight months ago. An ultrasound conducted during that time detected lesions in the uterine cavity. The lower abdomen scan revealed a small focal hyperechogenic area in the endometrial cavity suggestive of endometrial polyp. Kindly provide a diagnosis.
