A 38-year-old male presented with a chief complaint of headache and giddiness. He has a known case of Type II Diabetes mellitus and is currently under treatment for the same. His symptoms were indicative of a bilateral wide neck ICA-PCOM aneurysm. The patient's medical history revealed a history of Type II Diabetes mellitus and a history of smoking and alcohol use. There was no known drug allergy or illicit drug use. Upon evaluation, his complete blood counts were as follows: Hemoglobin level was 14.3 g/dL, total white blood cell count was 8500 cells/mm³, neutrophils accounted for 51.8%, and lymphocytes were at 36.2%. His platelet count was 236200. His random blood sugar levels were 141 mg/dL and 145 mg/dL in separate instances. His urea levels were 22.00 mg/dL and 16.00 mg/dL, and his creatinine level was 0.80 mg/dL. Prothrombin time was 10.6 seconds, while his glycated hemoglobin was 8.3%, indicating an estimated average glucose of 192 mg/dL. He was diagnosed with a bilateral wide neck ICA-PCOM aneurysm and informed about the nature of the disease, the recommended surgery, its advantages, disadvantages, consequences, and potential complications. After obtaining informed written consent, he underwent a right ICA endovascular flow diverter (FD) procedure on May 8, 2023. The post-operative period was uneventful, and he received analgesics and antibiotics. He was discharged on May 11, 2023, with prescribed medications: Tab. Brilinta, Tab. Ecosprin, Tab. Clonafit, Tab. Rosuvas, Tab. Ceftum, Tab. Gemer 2 mg, Tab. Jalra 100 mg, and Inj. Lantus subcutaneously at 12 units during the night. The patient was advised to review his condition in the Medicine OPD with Fasting Blood Sugar (FBS) and Postprandial Blood Sugar (PPBS) reports. Additionally, a follow-up was scheduled after one month for the left ICA flow diverter. Urgent care was recommended if he experienced fever, headache, or further giddiness. The patient and his relatives were informed about the surgery, and the patient's post-operative recovery was satisfactory.
