A 50-year-old female, with a history of obesity and diabetes for the past five years, is currently on a regimen of glimepiride 2 mg twice daily, metformin 1000 mg twice daily, and pioglitazone 15 mg once daily. Despite this combination, her HbA1c remains elevated at 8 percent. In an attempt to address the insufficient glycemic control, her treating physician introduced dapagliflozin 10 mg once daily, resulting in improved blood sugar levels for three months. Sometimes complaince is doubtful. So after taking all these measures if still blood sugars are uncontrollable then we need to introduce insulin
