A 58 year old female patient came to the emergency department at the hospital with the chief complaint of fever since morning, loss of consciousness since 2 hours along with left foot cellulitis 8*5 cm gangrenous tissue above ankle on lateral aspect and her RBS shows- 34 mg/dl with k/c/o DM-II and HTN since 8 years and on regular medication. The patient has a history of debridement in right leg cellulitis since 2 days. Patient vitals at the time of admission are the Temp- 101°F, Pulse- 88/min, BP- 116/70 mmHg, spo2- 98% on RA. L/E- Left lateral foot gangrene with bleeding+ just above the ankle, local temperature raised. S/E- CVS, RS, PA- NAD. Patient treated with Inj. D 25% 1 pint IV Stat. The patient is now conscious and oriented. Patient blood investigation s/o - Na: 126 mEq/L, K- 3.21 mEq/L, Cl- 86.50 mEq/L, Creat- 1.11 mg/dl, HB- 8.7 gm%, TLC- 14,540/cmm, plt- 4,40,000/cmm.
