A 34 year old female presented to casualty with shortness of breath, chest pain, abdominal pain, fever, rash over face, alopecia
She is known case of SLE on HCQ and low dose steroid stopped using since 1month.
With the history of recurrent DVT and consecutive 3 abortions she was diagnosed as APLA secondary to SLE
Inv- ANA - IIF 1:80,homogenous pattern
Anti ds DNA positive, lupus anticoagulant positive
DCT positive, thrombocytopenia, b/l pleural effusion, portal vein thrombus in ultrasound and CT abdomen
24hr urinary protein-1.2g/day
Patient improved with high dose glucocorticoid, IV immunoglobulin, anti coagulant
