A 54-year-old female patient with a history of rheumatoid arthritis (RA) for 20 years is referred by her rheumatologist for evaluation due to reduced vision and persistent discomfort in both eyes. Despite using preserved artificial tears every hour, she does not experience relief. Her RA is well-controlled systemically with disease-modifying therapy. Her visual acuity is measured at 20/40 in both eyes during the examination. The tear lake is virtually absent, and the cornea shows diffuse staining with occasional filaments. Schirmer testing reveals a measurement of 0 mm, indicating severely reduced tear production. The remaining aspects of the examination show no notable findings.
