🌟 A Journey of Hope and Healing: My Most Challenging LMCA PCI Case 🌟 As a cardiologist, I often encounter cases that test the limits of medical science and my own confidence. Recently, I treated a 75-year-old gentleman with ACS NSTEMI and LVEF of 35%, presenting with critical blockages: • LMCA stenosis: 80-90% • LCX ostium: 90% • RCA: 90% diffuse disease • A non-graftable LAD. To complicate things further, he was dealing with severe COPD and ongoing chest pain. CABG was off the table due to his age and comorbidities, leaving us with one risky option. I reached out to my mentor, Dr. Pravesh from KGMU, a legend in interventional cardiology. He acknowledged the high risk but encouraged me to proceed. With his words and the blessings of the Almighty, we took on this challenge. Using a dedicated LMCA bifurcation minicrush technique, we achieved a fantastic outcome. The patient was pain-free within days, and we later completed the stage PCI to RCA during his hospital stay. Yesterday, he walked into my OPD with a big Colgate smile, sharing that he can now enjoy his garden again. His EF is now at 50%, a remarkable improvement!
