A 41-year-old male, a known case of chronic calcific pancreatitis, presented with central and right sided chest pain. The CT chest as given in the images demonstrated mediastinal collection and right pleural effusion and right lower lobe consolidation. The BAL fluid as well as Pleural fluid amylase levels were found to be highly elevated. The MRCP showed presence of a pancreaticopleural fistula and mediastinal collection communicating via aortic hiatus. A diagnosis of Mediastinal collection due to pancreatitis was made which is a rare entity
